Specialty Programs

Diabetic

We provide diabetes care and education to people who have this disease, as well as, those who are having trouble controlling it. In collaboration with the patient’s physician – diet, medication, and lifestyle changes are taught to the patient and family to best manage this new way of life.

Balance and Fall Prevention

For our patients that are assessed as a fall risk, we provide this program to ensure that they are safe at home using our safe transitions assessment on each patient along with standardized clinical assessments, that are administered by licensed physical and occupational therapist. This allows us to create a more personalized discipline plans of care.
Heart Failure
Our congestive heart failure program was designed to ensure that our patients understand the disease process & symptoms of CHF. We educate our patients and their caregivers on their medications, establishing routines, effective habits to control and best manage their heart failure.
Wound
We recognize that many of our patients need specialized care. That’s why we have a group of nurses who are certified specifically to treat complex wounds. As part of our multi-disciplinary clinical team, our wound care nurses closely monitor wounds while providing patient education and treatment regimens to ensure wounds heal.
Orthopedic
Our rehabilitation team provides a well-structured, evidence-based rehabilitation program for patients recovering from orthopedic illness and injury, such as multiple orthopedic trauma, hip fracture surgery, joint replacements, and arthritis. Homecare physical and occupational therapists offer condition-specific treatments to improve patients’ strength, mobility, and balance.

Cardio-pulmonary

For patients with chronic conditions, such as heart failure, diabetes and chronic obstructive pulmonary disease (COPD), our Disease Management programs aim to help patients maintain independence and a healthier lifestyle. Services include nursing care, rehabilitation, patient education, and in-home patient monitoring.
Neurology
For patients recovering from a spine or brain surgery, CVA/stroke, Multiple Sclerosis, Parkinson’s or Alzheimers, our team works to restore their level of function and educate on the risk factors they can change, treat, or control to prevent any further decline in quality of life. Home modification recommendations are also provided to the patient and family to prevent unforeseen falls in the home. Services include nursing care, physical, occupational and speech therapies, medical social work, and certified nurse assistants.
COPD
Our interdisciplinary healthcare team develops a plan to manage and maintain each patient’s Chronic Obstructive Pulmonary Disease (COPD). This includes an action plan for “what to do” if they experience symptoms or flare-ups, medication management, breathing exercises, and to improve their overall quality of life.
Respiratory Therapy

We will work closely with your healthcare providers to develop a plan on treatment.
